Коморбидность хронической боли и депрессии у неврологических больных

Аннотация
Представлен обзор литературы, посвященной сочетанию хронических болевых синдромов и депрессии. Хроническая боль и депрессия оказывают взаимное влияние на клиническую манифестацию, течение и эффективность лечения у таких пациентов. При наличии множества неврологических, соматических и вегетативных расстройств, характерных для сочетания боли и депрессии, важное значение приобретает понимание природы этой коморбидности, а также ее диагностики и приоритетов в лечении. Сложность выявления собственно аффективной симптоматики у таких пациентов определяет менее благоприятные исходы, персистирующее течение хронической боли, развитие терапевтической резистентности и риск рецидивирования заболевания. Обсуждаются вопросы эпидемиологии, особенности клинических проявлений хронической боли и депрессии при наиболее распространенных неврологических заболеваниях. Рассматриваются современные концепции патогенеза коморбидности хронической боли и депрессии, вопросы психофармакологии.
