Клинико-функциональная оценка ремиссии у пациентов с рекуррентной депрессией

Цель исследования – клинико-шкальная оценка ремиссии у пациентов с рекуррентной депрессией.
Пациенты и методы. У 121 пациента с рекуррентной депрессией изучены клинико-функциональные характеристики ремиссии. Использовали следующие тесты: шкалу Монтгомери-Асберг для оценки депрессии (Montgomery–Аsberg Depression Rating Scale, MADRS), краткую батарею тестов для оценки когнитивных функций при аффективных расстройствах (Brief Assessment of Cognition in Affective Disorders, BAC-A), шкалу оценки уровня инвалидности ВОЗ (WHO Disability Assessment Schedule, WHODAS 2.0), шкалу для оценки качества жизни SF-36 (Short Form-36). Проводили систематизацию ремиссии с выделением четырех ее типов: бессимптомная, с астеническими симптомами и тревожными симптомами, гипотимная.
Результаты и обсуждение. У 58,6% больных наблюдалось соответствие критериям полной ремиссии при длительности ремиссии в среднем 6 мес. Все пациенты в состоянии ремиссии имели нарушения социального функционирования. Предикторами неполной ремиссии являлись: отягощенная наследственность; мужской пол; сложная структура депрессии; наличие коморбидной тревоги. Преобладал тревожный вариант неполной ремиссии, что свидетельствует о целесообразности выбора для поддерживающей терапии антидепрессантов с анксиолитическим компонентом действия. Терапия антидепрессантами новых поколений чаще ассоциирована с наличием полной ремиссии. Возможность достижения функциональной ремиссии при терапии различными антидепрессантами изучали на примере агомелатина.
Заключение. При оценке результатов лечения депрессии необходимо учитывать не только клиническую динамику состояния, но и социальное функционирование пациентов, в качестве мишени терапии выделять позитивный аффект как один из психологических параметров качества жизни. Позитивное влияние на социальное функционирование пациентов должно быть важным критерием при выборе антидепрессанта для поддерживающей терапии рекуррентной депрессии.
