Эпилептические припадки у больных с инсультом

Аннотация
Примерно у 10 % пациентов после инсульта развивается постинсультная эпилепсия. Учитывая значительное бремя инсульта во всем мире, постинсультную эпилепсию расценивают как серьезную проблему для выживших пациентов. Постинсультная эпилепсия является важнейшим фактором, определяющим прогноз инсульта, и связана с высоким риском смертности, неблагоприятными исходами и увеличением продолжительности госпитализации. Как ранние припадки, происходящие в первые 7 дней после развития инсульта, так и поздние припадки, возникающие более чем через 7 дней после начала инсульта, являются независимыми предикторами постинсультной эпилепсии. Патофизиологические механизмы развития ранних и поздних эпилептических припадков различны. Возникновение постинсультных припадков связано с рядом факторов, включая тип инсульта (ишемический или геморрагический), локализацию и размер поражения, тяжесть заболевания и др. Прогностические модели помогают оценить риск эпилептогенеза у пациентов после инсульта. Эффективность противоэпилептических препаратов при постинсультной эпилепсии выше, чем при других причинах фокальной эпилепсии.
