The investigation enrolled 82 people aged 40–59 years (41 patients with EH and 41 healthy individuals (a control group)). Twenty-four blood pressure monitoring and brain MRI were performed in different modes (T1 MPRAGE, T2 TSE, T2 FLAIR, and DTI). </p></sec><sec><title>Results</title><p>Results. White matter hyperintensities (WMHs) were found in 7.3% of the healthy individuals and in 53.7% of the hypertensive patients (p=0.0002). The latter had significantly lower fractional anisotropy (FA) values in the white matter of the left inferior frontal gyrus than the healthy individuals (0.39±0.06 and 0.45±0.09, respectively; p&lt;0.001). FA was lower in the hypertensive patients than in the healthy individuals not only in the presence of WMHs (the left inferior frontal gyrus white matter was 0.397±0.071 and 0.45±0.09, respectively; p=0.009; the genu of the corpus callosum was 0.79±0.04 and 0.81±0.05, respectively; p=0.045), but also in the absence of WMHs (the left inferior frontal gyrus white matter was 0.378±0.073 and 0.45±0.09, respectively; p=0.0007). Discussion. The treatment-naХve patients with uncomplicated grade 1–2 EH with short-term (2,3 year) duration were found to have significantly lower FA values in the left inferior frontal gyrus white matter than the healthy normotensive subjects of the same age. Thus, the microstructural integrity of white matter is impaired in middle-aged hypertensive patients even at the earliest disease stages. </p></sec><sec><title>Conclusion</title><p>Conclusion. Middle-aged treatment-naive patients with uncomplicated grade 1–2 EH have lower FA in the left inferior frontal gyrus white matter even in the absence of WMHs. </p></sec></abstract><trans-abstract xml:lang="ru"><p>Диффузионно-тензорная магнитно-резонансная томография (ДТ-МРТ) – единственный неинвазивный метод, позволяющий прижизненно изучать микроструктуру белого вещества головного мозга и количественно оценивать полученные изображения. </p><p>Цель исследования – изучение белого вещества головного мозга при помощи ДТ-МРТ у нелеченых пациентов среднего возраста с неосложненной эссенциальной артериальной гипертензией (АГ) 1–2-й степени. </p><sec><title>Пациенты и методы</title><p>Пациенты и методы. В исследование включено 82 человека в возрасте 40–59 лет (41 пациент с эссенциальной АГ и 41 здоровый испытуемый – контрольная группа). Проведены суточное мониторирование артериального давления и МРТ головного мозга в разных режимах (Т1 MPRAGE, T2 TSE, T2 FLAIR, DTI). </p></sec><sec><title>Результаты</title><p>Результаты. Гиперинтенсивные изменения (ГИ) белого вещества головного мозга выявлены у 7,3% здоровых лиц и у 53,7% пациентов с АГ (р=0,0002). У пациентов с АГ по сравнению со здоровыми показатели фракционной анизотропии (ФА) в белом веществе нижней лобной извилины были статистически значимо меньше (0,39±0,06 и 0,45±0,09; р&lt;0,001 соответственно). ФА была ниже у пациентов с АГ, чем у здоровых, не только при наличии ГИ белого вещества головного мозга (белое вещество нижней лобной извилины – 0,397±0,071 и 0,45±0,09 соответственно; р=0,009; колено мозолистого тела – 0,79±0,04 и 0,81±0,05 соответственно; р=0,045), но и при отсутствии этих изменений (белое вещество нижней лобной извилины – 0,378±0,073 и 0,45±0,09 соответственно; p=0,0007). Обсуждение. У нелеченых больных с неосложненной АГ 1–2-й степени с небольшой (2,3 года) длительностью заболевания обнаружены достоверно меньшие значения ФА в белом веществе нижней лобной извилины слева по сравнению со здоровыми лицами той же возрастной группы с нормальным АД. Следовательно, у пациентов среднего возраста с АГ микроструктурная целостность белого вещества нарушена уже на самых ранних этапах заболевания.</p></sec><sec><title>Заключение</title><p>Заключение.
