White matter hyperintensive changes were found in 7.5% of the healthy individuals and in 51.5% of the hypertensive patients (p = 0.0002). In hypertensive patients, CBF in the cortical plate of anterior frontal regions was significantly (p &lt; 0.001) lower than that in the controls: right CBF, 39.1±5.6 and 45.8±3.2 ml/100 g/min, respectively; left CBF, 39.2±6.2 and 45.2±3.6 ml/100 g/min, respectively. In hypertensive patients with white matter hyperintensive changes, CBF was significantly lower than that in the controls: right CBF, 38.5±5.9 ml/100 g/min (p = 0.0001); left CBF, 39.2±6.7 ml/100 g/min (p = 0.002), and in those without these changes, right CBF was 39.5±5.1 ml/100 g/min (p = 0.0002); left CBF was 38.9±4.3 ml/100 g/min (p = 0.00002). Correlation analysis revealed significant inverse correlations of CBF with BP and systolic BP variability.</p></sec><sec><title>Conclusion</title><p>Conclusion. Lower cerebral perfusion occurs in middle-aged untreated patients with uncomplicated grade 1–2 hypertension even in the absence of white matter hyperintensity foci. </p></sec></abstract><trans-abstract xml:lang="ru"><p>Метод спиновой маркировки артериальной крови (Arterial Spin Labeling, ASL) – перспективный неинвазивный метод оценки перфузии головного мозга, который позволяет выявить снижение церебрального кровотока (Сerebral Blood Flow, СBF).</p><p>Цель исследования – оценка перфузии головного мозга у нелеченых пациентов среднего возраста с неосложненной артериальной гипертензией (АГ) 1–2-й степени в сравнении с контрольной группой здоровых лиц того же возраста.</p><sec><title>Пациенты и методы</title><p>Пациенты и методы. Обследовано 33 больных эссенциальной АГ и 40 здоровых лиц (контрольная группа) в возрасте 40–59 лет. Проведены суточное мониторирование артериального давления (АД) и магнитно-резонансная томография головного мозга в разных режимах (Т1 MPRAGE, T2 TSE, T2 FLAIR, DTI, ASL).</p></sec><sec><title>Результаты</title><p>Результаты. Гиперинтенсивные изменения белого вещества головного мозга выявлены у 7,5% здоровых и 51,5% больных АГ (р=0,0002). У больных АГ СBF в кортикальной пластинке передних лобных отделов головного мозга оказался достоверно (р&lt;0,001) ниже, чем в норме: справа – 39,1±5,6 и 45,8±3,2 мл/100 г/мин соответственно, слева – 39,2±6,2 и 45,2±3,6 мл/100 г/мин соответственно. У больных АГ с наличием гиперинтенсивных изменений белого вещества головного мозга CBF был достоверно ниже, чем в норме: cправа – 38,5±5,9 мл/100 г/мин (p=0,0001), слева – 39,2±6,7 мл/100 г/мин (p=0,002), а при отсутствии этих изменений этот показатель составил справа 39,5±5,1 мл/100 г/мин (p=0,0002), слева – 38,9±4,3 мл/100 г/мин (p=0,00002). Корреляционный анализ выявил достоверные обратные взаимосвязи CBF с уровнем АД и вариабельностью систолического АД.</p></sec><sec><title>Заключение</title><p>Заключение.
