However, by taking into account the multifactorial genesis of POCD and the involvement of mechanisms, such as neuroinflammation and apoptosis, in its development, the additional use of neuroprotective agents seems justified. Further investigations are needed to develop effective therapeutic strategies for treating POCD. This review gives the data currently available in the literature on the clinical phenomenology, pathogenesis, treatment, and pharmacological prevention of POCD.</p></abstract><trans-abstract xml:lang="ru"><p>Послеоперационная когнитивная дисфункция (ПОКД) – синдром когнитивных нарушений (КН) вследствие общей анестезии и хирургического вмешательства, имеющий мультифакторную этиологию. Актуальность проблемы профилактики и коррекции ПОКД продиктована ее высокой частотой, особенно у пожилых пациентов, и долгосрочными последствиями, ухудшающими качество жизни. До настоящего времени вопрос о возможности предотвращения ПОКД остается дискуссионным, не существует общепринятой эффективной стратегии снижения ее частоты. Ряд предлагаемых методов, направленных на снижение риска развития КН, сводится, в частности, к улучшению операционных и анестезиологических техник с переходом на малоинвазивные вмешательства и использование общих анестетиков с коротким действием. Однако, учитывая мульфакторный генез ПОКД и участие в ее развитии таких механизмов, как нейровоспаление и апоптоз, представляется оправданным также использование агентов с нейропротекторными эффектами.
