Objective: to study the prevalence of vital exhaustion (VI) and sleep disorders, the pattern of the latter in 45–69-year-old Novosibirsk dwellers (Siberia) with different levels of VI.
Patients and methods. A random representative sample of 45–69-year-old women (n=2401) from two Novosibirsk districts were examined within the 2003–2005 HAPIEE (Health, Alcohol and Psychosocial Factors In Eastern Europe) prospective cohort study. Another random representative sample of 45–69 men (n=1770) from two Novosibirsk districts were examined within a subsequent screening study. The entire sample consisted of 4171 residents: men and women were 42.4% and 57.6%, respectively. The mean age of the men was 56.5±7.01 and that of the women was 56.3±7.07. The response rate was as high as 61%. Testing was carried out using the short 14-item version of the Maastricht Questionnaire (MQ) to evaluate VE and the 4-item Jenkins Sleep Questionnaire (JSQ) to investigate sleep quality and duration.
Results. The leading determinants of VE were the incidence of fatigue, interrupted sleep, lethargy, the degree of irritability, and a feeling of exhaustion and tiredness. The population examined was found to have a high prevalence of VE and sleep disorders. There was a close correlation between high VE and impaired sleep quality and duration.
Conclusion. The high rate of impaired sleep quality and duration in the Novosibirsk population, which is sometimes twice greater than that in other populations, is associated with the high prevalence of depression, anxiety, VE, and hostility among the population. This is due to the high prevalence of cardiovascular diseases. A sleep disorder may cause VE. It is important to promptly establish a psychotherapeutic diagnosis of these conditions, followed by their subsequent correction.
Цель исследования – изучение распространенности жизненного истощения (ЖИ) и нарушений сна, структуры нарушений сна у жителей Сибири (Новосибирск) в возрасте 45–69 лет с разным уровнем ЖИ.
Пациенты и методы. В рамках проспективного когортного исследования HAPIEE (Health, Alcohol and Psychosocial Factors In Eastern Europe) в 2003–2005 гг. обследована случайная репрезентативная выборка женского населения 45–69 лет двух районов Новосибирска (n=2401). В рамках следующего скринирующего исследования обследована случайная репрезентативная выборка мужского населения 45–69 лет двух районов Новосибирска (n=1770). Вся выборка составила 4171 жителей: мужчин – 42,4%, женщин – 57,6%. Средний возраст мужчин составил 56,5±7,01 года, женщин – 56,3±7,07 года. Отклик достигал 61%. Тестирование проводили с помощью вопросников The Maastricht Questionnaire (MQ; short version, 14-item) для оценки ЖИ (Vital Exhaustion) и C.D. Jenkins и соавт. «4-item Jenkins Sleep Questionnaire» (JSQ) – для изучения нарушений качества и продолжительности сна.
Результаты. Ведущими факторами, определяющими ЖИ, были: частота возникновения усталости, прерывистый сон, недостаток сил, степень раздражения, чувство изнеможения и усталости. Установлена высокая распространенность ЖИ и нарушенийсна в обследованной популяции. Выявлена тесная связь высокого уровня ЖИ с нарушениями как качества, так и продолжительности сна.
Заключение. Высокая частота в новосибирской популяции нарушений качества и продолжительности сна, превышающая иногда в 2 раза эти показатели в других популяциях, связана с высоким распространением среди населения депрессии, тревоги, ЖИ, враждебности. Следствием этого является высокая распространенность сердечно-сосудистых заболеваний. Расстройство сна может быть причиной ЖИ. Важно своевременно проводить психотерапевтическую диагностику этих состояний с последующей их коррекцией.
