These treatments for IS make it possible to expand indications for revascularization and to create prerequisite for improving the outcomes of treatment.</p><p>Interventional treatments for IS should not be set off against intravenous thrombolytic therapy (ITT); it is necessary to seek their reasonable sharing. The promising directions in the development of ITT and endovascular revascularization (EVR) for IS are to upgrade tools and to choose the safest and most effective constructions, as well as to identify target patient groups needing these treatment options based on the prediction of the efficacy of these techniques with consideration for clinical findings, the pattern of a lesion, and radiological data (including estimates of perfusion, ischemic core-penumbra relationships, and collateral blood supply intensity).</p><p>The paper discusses the present status and prospects of EVR of cerebrovascular arteries, its indications, and basic procedures.</p></abstract><trans-abstract xml:lang="ru"><p>Внутрисосудистые методы лечения ишемического инсульта (ИИ) начинают входить в клиническую практику и, как показывают результаты недавних исследований, имеют хорошие перспективы. Применение внутрисосудистых методов лечения ИИ позволяет расширить показания к реваскуляризации, создает предпосылки для улучшения исходов лечения.</p><p>Интервенционные методы лечения ИИ не стоит противопоставлять внутривенной тромболитической терапии (ВТТ) – нужно стремиться к их обоснованному совместному использованию. Перспективными направлениями развития методов ВТТ и внутрисосудистой реваскуляризации (ВСР) при ИИ являются как совершенствование инструмента и отбор наиболее безопасных и эффективных конструкций, так и выделение целевых групп пациентов, нуждающихся в этих способах лечения, на основании прогнозирования эффективности применения этих методов с учетом клинических данных, характера поражения, данных лучевых исследований (в том числе оценки перфузии, характера взаимоотношений зоны ишемического ядра и пенумбры, выраженности коллатерального кровоснабжения).</p><p>В статье обсуждаются современное состояние и перспективы ВСР церебральных артерий, показания к ее проведению и основные методики.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>ишемический инсульт</kwd><kwd>внутрисосудистая реваскуляризация</kwd><kwd>тромбэктомия</kwd><kwd>интраартериальный тромболизис</kwd></kwd-group><kwd-group xml:lang="en"><kwd>ischemic stroke</kwd><kwd>endovascular revascularization</kwd><kwd>thrombectomy</kwd><kwd>intra-arterial thrombolysis</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Шамалов НА.
