Dizziness in stroke is usually accompanied by other focal neurological symptoms of brainstem and cerebellar involvement. However, stroke may appear as isolated vestibular vertigo in some cases. An analysis of history data and the results of neurovestibular examination and brain magnetic resonance imaging allows stroke to be diagnosed in patients with acute isolated dizziness. The treatment of patients with stroke-induced dizziness involves a wide range of medications for the reduction of the degree of dizziness and unsteadiness and for the secondary prevention of stroke. Vestibular rehabilitation is an important component of treatment. The paper describes an observation of a patient with poorly controlled hypertension, who developed new-onset acute systemic dizziness. Vestibular neuronitis might be presumed to be a peripheral cause of vestibular disorders, by taking into account the absence of additional obvious neurological symptoms (such as pareses, defective sensation, diplopia, etc.) and the nature of nystagmus. However, intention tremor in fingernose and heel-knee tests on the left side, a negative Halmagyi test, and results of Romberg’s test could suggest that stroke was a cause ofdizziness.</p></abstract><trans-abstract xml:lang="ru"><p>Дифференциальная диагностика острого впервые возникшего вестибулярного головокружения, проводится прежде всего между вестибулярным нейронитом и инсультом. Головокружение при инсульте обычно сопровождается другими очаговыми неврологическими симптомами поражения ствола мозга и мозжечка. Однако в некоторых случаях инсульт может проявляться изолированным вестибулярным головокружением. Диагностировать инсульт у пациентов с острым изолированным головокружением позволяет анализ данных анамнеза, результатов нейровестибулярного исследования, магнитно-резонансной томографии головного мозга. Лечение пациентов с головокружением, вызванным инсультом, включает широкий спектр лекарственных препаратов, направленных на уменьшение выраженности головокружения и неустойчивости, а также вторичную профилактику инсульта. Важным компонентом лечения является вестибулярная реабилитация. Представлено наблюдение пациента, страдающего плохо контролируемой артериальной гипертензией, у которого остро впервые в жизни развилось системное головокружение. Учитывая отсутствие дополнительной очевидной неврологической симптоматики (например, парезов, расстройств чувствительности, двоения и т. д.) и характер нистагма, можно было бы предположить периферический характер вестибулярных нарушений, например вследствие вестибулярного нейронита.
