CCVA appears as cognitive impairments (CIs), affective (emotional) disorders and focal syndromes. Treatment for CCVA requires a comprehensive approach. Effective combination therapy for CCVA involves secondary prevention of stroke and CIs; treatment of CIs; treatment of depression and other affective disorders; and neuroprotective therapy. Basic therapy for CCVA includes modification of risk factors, antihypertensive, hypolipidemic, and antithrombotic therapies. Central acetylcholinesterase inhibitors (galantamine, rivastigmine, donepezil) and a reversible NMDA receptor blocker (memantine) are symptomatically used at a stage of vascular and mixed dementia. There are no unique guidelines for the therapy of mild and moderate vascular nondementia-related CIs. Drug use, based on the neurochemical mechanisms underlying the development of vascular CIs, is substantiated. When choosing psychotropic agents, it is necessary to take into account the causes and clinical manifestations of neuromediator deficiency. Antidepressants are used as essential drugs. Neuroleptics and tranquilizers are additionally administered in complex-pattern syndromes, such as depression with marked anxiety. Prescription of neuroprotectors may be effective in treating both stroke and CCVA. These medicaments are most effective when a damaging factor acts, i.e. neuroprotectors should be given in a risk situation and to reduce damage. Citicoline is one of the most test drugs in a group of neuroprotectors. </p></abstract><trans-abstract xml:lang="ru"><p>Хроническое нарушение мозгового кровообращения (ХНМК) – синдром поражения головного мозга, вызванный сосудистыми факторами. ХНМК проявляется когнитивными нарушениями (КН), аффективными (эмоциональными) расстройствами и очаговыми синдромами. Лечение ХНМК требует комплексного подхода. Комплексная эффективная терапия ХНМК включает вторичную профилактику инсульта и КН; лечение КН; лечение депрессии и других аффективных расстройств; нейропротективную терапию. Базисная терапия ХНМК включает в себя модификацию факторов риска, антигипертензивную, гиполипидемическую и антитромботическую терапию. На стадии сосудистой и смешанной деменции с симптоматической целью применяются ингибиторы центральной ацетилхолинэстеразы (галантамин, ривастигмин, донепизил) и обратимый блокатор NMDA-рецепторов мемантин. Отсутствуют однозначные рекомендации по терапии сосудистых недементных (легких и умеренных) КН. Обоснованным является использование препаратов, исходя из нейрохимических механизмов, лежащих в основе развития сосудистых КН. При подборе психотропных средств нужно учитывать причины и клинические проявления дефицита нейромедиаторов. В качестве основных средств используются антидепрессанты. При синдромах сложной структуры, например при сочетании депрессии с выраженной тревогой, дополнительно применяются нейролептики и транквилизаторы. Назначение нейропротекторов может быть эффективным как при острых нарушениях мозгового кровообращения, так и при ХНМК. Эти препараты наиболее эффективны во время действия повреждающего фактора, т. е. нейропротекторы следует назначать в ситуации риска и для уменьшения повреждения.
