The article describes the modern concepts of etiology, pathogenetic mechanisms, clinical features and approaches to diagnosis and therapy of vascular cognitive impairment (VCI). </p><p>Cerebrovascular accident, chronic cerebral circulatory insufficiency and their combination, sometimes in combination with a concomitant neurodegenerative process, are shown to be the major types of brain lesions leading to VCI. The clinical presentation of VCI is characterized by the neuropsychological status dominated by impairment of the executive frontal functions (planning, control, attention) in combination with focal neurological symptoms. </p><p>The diagnosis is based on comparing of the revealed neuropsychological and neurological features with neuroimaging data. Neurometabolic, acetylcholinergic, glutamatergic, and other vasoactive drugs and non-pharmacological methods are widely used to treat VCI. </p></abstract><trans-abstract xml:lang="ru"><p>Сосудистая патология головного мозга является второй по распространенности причиной когнитивных нарушений (КН) после болезни Альцгеймера. В статье изложены современные представления об этиологии, патогенетических механизмах, клинических особенностях и подходах к диагностике и терапии сосудистых КН. Показано, что основными видами поражения головного мозга, которые приводят к КН, являются инсульт, хронические формы недостаточности кровоснабжения головного мозга и их сочетание, иногда в комбинации с сопутствующим нейродегенеративным процессом. Клиническая картина сосудистых КН характеризуется преобладанием в нейропсихологическом статусе нарушения управляющих лобных функций (планирование, контроль, внимание) в сочетании с очаговой неврологической симптоматикой. Диагноз базируется на сопоставлении выявляемых нейропсихологических и неврологических особенностей с данными нейровизуализации.
