Acute vestibular vertigo (AVV) is one of the most common reasons why patients seek medical advice from doctors of various specialities. Concomitant neck pain (cervicalgia) often leads to a misdiagnosis of 'cervical vertigo' in patients with benign paroxysmal positional vertigo (BPPV). This article presents a clinical case of a 41-year-old patient with concurrent BPPV and cervicalgia. Due to the initial misdiagnosis of 'cervical vertigo', the patient remained unable to work for 6 months. The timely identification of myofascial syndrome and BPPV enabled effective pathogenetic treatment and ensured the patient's rapid return to work. The effectiveness of a fixed combination of dimenhydrinate and cinnarizine (the drug Arlevert) in various aetiological factors of AVV is discussed. This combination has a minimal sedative effect compared with other vestibulolytics and contributes to the rapid relief of the intensity of vertigo and autonomic symptoms.

Острое вестибулярное головокружение (ОВГ) является одной из наиболее частых причин обращения пациентов к врачам разных специальностей. Сопутствующая боль в шее (цервикалгия) нередко приводит к ошибочной диагностике «шейного головокружения» у пациентов с доброкачественным пароксизмальным позиционным головокружением (ДППГ). В статье представлен клинический случай пациента 41 года с сочетанным развитием ДППГ и цервикалгии. Вследствие первоначальной ошибочной диагностики «шейного головокружения» пациент оставался нетрудоспособным в течение 6 мес. Своевременное выявление миофасциального синдрома и ДППГ позволило провести эффективную патогенетическую терапию и обеспечить быстрое возвращение пациента к профессиональной деятельности. Рассматривается эффективность применения фиксированной комбинации дименгидрината и циннаризина (препарат Арлеверт) при различных этиологических факторах ОВГ. Данная комбинация обладает минимальным седативным эффектом по сравнению с другими вестибулолитиками и способствует быстрому купированию интенсивности головокружения и вегетативной симптоматики.
