The aetiology of pain in NF1 is multifactorial and not fully understood, which complicates the selection of effective treatment. Current treatment methods – pharmacotherapy, surgical removal of tumours and neuromodulation – do not always provide a lasting analgesic effect and are associated with a risk of complications or recurrence of pain. The clinical case presented demonstrates the successful treatment of severe, drug-resistant pain syndrome in a patient with NF1 using botulinum toxin type A (incobotulinumtoxin). The use of botulinum toxin type A under ultrasound guidance resulted in a significant reduction in pain intensity, a decrease in the frequency of pain episodes, and an improvement in the patient’s quality of life, with no adverse events. Thus, we report for the first time the use of incobotulinumtoxin in NF1 with a positive clinical outcome.</p></abstract><trans-abstract xml:lang="ru"><p>Хронический болевой синдром остается одной из наиболее сложных проблем ведения пациентов с нейрофиброматозом 1-го типа (НФМ1), значительно снижая качество их жизни. Этиология боли при НФМ1 многофакторна и до конца не ясна, что затрудняет выбор эффективной терапии. Имеющиеся на сегодняшний день методы лечения – фармакотерапия, хирургическое удаление опухолей и нейромодуляция – не всегда обеспечивают стойкий анальгетический эффект и сопряжены с риском осложнений или рецидива боли. В представленном клиническом случае продемонстрирована успешная терапия тяжелого фармакорезистентного болевого синдрома у пациентки с НФМ1 с помощью ботулинического токсина типа А (инкоботулотоксин). Применение ботулинического токсина типа А под ультразвуковым контролем позволило добиться значимого снижения интенсивности боли, уменьшения частоты болевых пароксизмов и улучшения качества жизни пациентки при отсутствии нежелательных явлений.
