The development of dementia is preceded by mild CI (MCI) over several years.</p><sec><title>Objective</title><p>Objective: to investigat the effect of inhaled nitric oxide therapy on cognitive function (CF) in patients with MCI due to CVD and NDD.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 94 patients (25 men and 69 women aged 50 to 76 years, mean age 65.3±8.9 years) with MCI due to CVD and/or NDD of the brain. The patients were randomized into two groups: the main group (therapy) and the control group. The control group comprised 22 patients with MCI (5 men and 17 women aged 50 to 76 years, mean age 62.3±8.5 years) who received standard therapy (antihypertensive, antithrombotic and other agents). The main group comprised 72 patients (20 men and 52 women aged 50 to 76 years, mean age 66.2±8.0 years) who additionally inhaled a nitric oxide-enriched air mixture for 30 minutes daily using the Tianox device. Each patient had from 7 to 10 inhalation sessions (the first trial session lasted 20 minutes, the following sessions 30 minutes). Neuropsychological tests, emotional state and sleep quality were examined at the beginning and 3 and 6 months after the end of treatment. Results. In the main group, after 3 and 6 months, a significant (p&lt;0.05) improvement in CF was found according the Montreal Cognitive Assessment (MoCA-test), Frontal Assessment Battery, the Digit Forward and Backward Span Test, the Digit Symbol Substitution Test, the Munsterberg Test, the Literal Association Test and the 12-word Delayed Free Recall Test. A significant (p&lt;0.05) decrease in anxiety and depression according to the Hospital Anxiety and Depression Scale, the Beck Depression Inventory and the Spielberger Anxiety Scale as well as an improvement in sleep quality according to the Insomnia Severity Index were also observed. The therapy was well tolerated, and no serious adverse events occurred. No significant improvement in CF, emotional state or sleep was observed in the control group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Brief therapy (7–10 sessions) with inhaled nitric oxide improves the CF and emotional state of patients with MCI on a background of CVD and NDD. Further studies on inhaled nitric oxide therapy in patients with MCI in a multicenter randomized trial with sham treatment are needed.</p></sec></abstract><trans-abstract xml:lang="ru"><p>Хронические цереброваскулярные заболевания (ЦВЗ) и нейродегенеративные заболевания (НДЗ) являются самыми частыми причинами когнитивных нарушений (КН). Развитию деменции на протяжении нескольких лет предшествуют умеренные КН (УКН).</p><p>Цель исследования – изучение влияния терапии ингаляционным оксидом азота на когнитивные функции (КФ) пациентов с УКН вследствие ЦВЗ и НДЗ.</p><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 94 пациента (25 мужчин и 69 женщин в возрасте от 50 до 76 лет, средний возраст – 65,3±8,9 года) с УКН вследствие ЦВЗ и/или НДЗ головного мозга. Пациенты были рандомизированы на две группы: основную (терапевтическую) и контрольную. В контрольную группу были включены 22 пациента с УКН (пять мужчин и 17 женщин в возрасте от 50 до 76 лет, средний возраст – 62,3±8,5 года), которые получали стандартную терапию (антигипертензивные, антитромботические и другие средства). В основную группу были включены 72 пациента (20 мужчин и 52 женщины в возрасте от 50 до 76 лет, средний возраст – 66,2±8,0 года), которые дополнительно получали ежедневные ингаляции воздушной смеси, обогащенной оксидом азота, в течение 30 мин на аппарате «Тианокс». Пациенты проходили от 7 до 10 сеансов ингаляций (первый, пробный, – 20 мин, последующие – 30 мин). Исходно и через 3 и 6 мес после окончания лечения были проведены нейропсихологическое тестирование, оценка эмоционального состояния и качества сна.</p></sec><sec><title>Результаты</title><p>Результаты. В основной группе через 3 и 6 мес получено значимое (р&lt;0,05) улучшение КФ по Монреальской когнитивной шкале, Батарее тестов для оценки лобной дисфункции, тесту повторения цифр в прямом и обратном порядке, тесту символьно-цифрового кодирования, Мюнстербергскому тесту, тесту на литеральные ассоциации, а также отсроченному свободному воспроизведению в тесте «12 слов». Также было отмечено значимое (р&lt;0,05) уменьшение проявлений тревоги и депрессии по Госпитальной шкале тревоги и депрессии, Шкале депрессии Бека, Шкале тревожности Спилбергера и улучшение качества сна по Индексу тяжести инсомнии. Отмечены хорошая переносимость терапии, отсутствие серьезных нежелательных явлений. В контрольной группе не наблюдалось существенного улучшения КФ, эмоционального состояния и сна.</p></sec><sec><title>Заключение</title><p>Заключение. Короткий курс (7–10 сеансов) терапии ингаляционным оксидом азота улучшает КФ и эмоциональное состояние пациентов с УКН на фоне ЦВЗ и НДЗ.
