As the frequency of attacks increases, so does the frequency of painkillers usage, and 85% of patients with CM are also diagnosed with medication overuse headache (MOH).</p><sec><title>Objective</title><p>Objective: to investigate the efficacy of preventive therapy of CM with or without MOH.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study involved 117 patients with CM and MOH (group 1) and 23 patients with CM without MOH (group 2), patients were examined at the initial visit and after 3 and 9 months of follow-up. All patients in the 1st group (CM+MOH) received a 7-day "detoxification" therapy. Patients in both groups were given an educational and information session on the type of their diagnosis, treatment methods and lifestyle correction. The choice of a preventive treatment in both groups was based on the current clinical recommendations, algorithms of modern international guidelines, concomitant pathology, the presence of contraindications in a particular patient, tolerability, as well as territorial and economic factors.</p></sec><sec><title>Results</title><p>Results. A positive effect was found in both groups, regardless of the presence of MOH. The lack of significant differences between the groups of CM with/without MOH is probably due to the use of "detoxification" therapy and the cessation/restriction of the use of the drug of abuse in all patients with CM and MOH, although a more pronounced positive dynamics is observed in patients of the 2nd group (CM without MOH), which in turn emphasizes the severity of the disease, the greater disability and the poorer prognosis for response to preventive treatment in the presence of MOH. It was found that one third of patients in the study favored monoclonal antibodies (mAbs) due to convenience, rapid onset of action and tolerability. In the group of patients with MOH, the best results were obtained when using injection methods of treatment (anti-CGRP mAbs, botulinum therapy) or when using combined therapy compared to monotherapy with venlafaxine or topiramate.</p></sec><sec><title>Conclusion</title><p>Conclusion. All first-line drugs for the CM prophylactic treatment according to the clinical guidelines of the Ministry of Health of Russia are clearly effective, including cases with both CM and MOH. The use of combined therapy in CM with/without MOH can be effective even with a low initial response to one of the monotherapies.</p></sec></abstract><trans-abstract xml:lang="ru"><p>От хронической мигрени (ХМ) страдают три человека из ста. При увеличении частоты приступов нарастает частота приема обезболивающих препаратов, и у 85% пациентов с ХМ параллельно диагностируется лекарственно-индуцированная головная боль (ЛИГБ).</p><p>Цель исследования – изучение эффективности профилактической терапии ХМ с ЛИГБ и без нее.</p><sec><title>Материал и методы</title><p>Материал и методы. В ходе исследования обследовано в момент первичного обращения, а также через 3 и 9 мес 117 пациентов c ХМ и ЛИГБ (1-я группа) и 23 пациента с ХМ без ЛИГБ (2-я группа). Все пациенты 1-й группы (ХМ+ЛИГБ) получили курс «детоксикационной» терапии в течение 7 дней. Для пациентов обеих групп проведена образовательно-информационная сессия о природе диагноза и методах лечения и коррекции образа жизни. Выбор средства для профилактического лечения в обеих группах проводился на основании действующих клинических рекомендаций, алгоритмов современных международных руководств, сопутствующей патологии, наличия противопоказаний у конкретного пациента, переносимости, а также территориальных и экономических факторов.</p></sec><sec><title>Результаты</title><p>Результаты. Показано, что положительный эффект наступал в обеих группах, вне зависимости от наличия ЛИГБ. Отсутствие значимых различий между группами ХМ с/без ЛИГБ, вероятно, связано с применением «детоксикационной» терапии и отменой / ограничением использования абузусного препарата у всех пациентов с ХМ и ЛИГБ. Однако более отчетливая положительная динамика наблюдается у пациентов 2-й группы (ХМ без ЛИГБ), что еще раз подчеркивает тяжесть состояния, большую инвалидизацию и худший прогноз отклика на профилактическое лечение при наличии ЛИГБ. Выявлено, что треть пациентов в исследовании отдавали предпочтение моноклональным антителам (мАТ) ввиду удобства применения, скорости наступления эффекта, переносимости. В группе пациентов с ЛИГБ лучший результат наблюдался при использовании инъекционных методов лечения (антиCGRP мАТ, ботулинотерапия) или при использовании комбинированного варианта терапии по сравнению с монотерапией венлафаксином или топираматом.</p></sec><sec><title>Заключение</title><p>Заключение. Все препараты первого выбора для профилактического лечения ХМ согласно клиническим рекомендациям Минздрава России значимо эффективны, в том числе при сочетании ХМ с ЛИГБ.
