The development and progression of psychotic symptoms in patients with PD is due to a combination of exogenous and endogenous mechanisms, such as an imbalance of neurotransmitter systems, the effect of antiparkinsonian drugs, individual characteristics of the disease course and structural neurodegenerative changes in the brain. Given the heterogeneity of psychotic symptoms and the lack of standardized diagnostic criteria, issues of timely identification and choice of therapeutic tactics are important and require special attention from neurologists and psychiatrists in clinical practise. This review highlights modern ideas on phenomenology, risk factors, pathogenesis and therapeutic approaches.</p></abstract><trans-abstract xml:lang="ru"><p>Психотические нарушения при болезни Паркинсона — распространенные немоторные проявления, оказывающие негативное влияние на качество жизни пациентов и ассоциированные с неблагоприятным прогнозом течения болезни. В основе развития и прогрессирования психотических симптомов у пациентов с болезнью Паркинсона лежит сочетание экзогенных и эндогенных механизмов, таких как дисбаланс нейромедиаторных систем, действие противопаркинсонических препаратов, индивидуальные особенности течения заболевания и структурные нейродегенеративные изменения головного мозга. Учитывая гетерогенность психотических симптомов и отсутствие стандартизированных диагностических критериев, вопросы своевременного выявления и выбора терапевтической тактики являются актуальными и требуют особого внимания неврологов и психиатров в клинической практике.
