Currently, serious adverse reactions (ARs) are extremely rare. The more common mild ARs that can develop during and after stimulation remain less well understood but may affect procedure tolerance and treatment adherence.</p><sec><title>Objective</title><p>Objective: to study the tolerability and safety of TMS in patients and healthy volunteers.</p></sec><sec><title>Material and methods</title><p>Material and methods. In a retrospective cohort study using standardized questionnaires, ARs were assessed during and within 24 hours after TMS in patients (n=103) and healthy volunteers (n=74).</p></sec><sec><title>Results</title><p>Results. In total, the data of 1246 sessions were analyzed. ARs were observed during 56.8% of the analyzed sessions and within 24 hours after in 21.5% of the sessions. In most cases, they were mild and did not affect the intent to continue stimulation. TMS withdrawal due to ARs was reported in six cases (3.3% of participants), three of which experienced severe headache (HA) after stimulation. During stimulation, sleepiness (32.4%) and pain in the stimulation area (20.3%) were most often recorded. The most frequent AR within 24 hours after TMS was HA (13.3% of sessions), its characteristics are described and possible diagnostic criteria are proposed.</p></sec><sec><title>Conclusion</title><p>Conclusion. ARs can occur during and after a TMS session and are mild in most cases. TMS-induced HA, which is the most common AR within 24 hours after TMS, can be considered a distinct type of secondary HA, the pathogenesis of which requires further study.</p></sec></abstract><trans-abstract xml:lang="ru"><p>Транскраниальная магнитная стимуляция (ТМС) находит все более широкое применение в исследовательской и клинической практике, поэтому особенно важными являются вопросы ее безопасности и переносимости. В настоящее время серьезные нежелательные эффекты (НЭ) являются крайне редкими. Более распространенные легкие НЭ, которые могут развиваться во время и после стимуляции, остаются менее изученными, однако могут влиять на переносимость процедуры и приверженность лечению.</p><p>Цель исследования – изучение переносимости и безопасности ТМС у пациентов и здоровых добровольцев. Материал и методы. В рамках ретроспективного когортного исследования с помощью стандартизированных опросников оценивались НЭ во время и в течение 24 ч после ТМС у пациентов (n=103) и здоровых добровольцев (n=74).</p><sec><title>Результаты</title><p>Результаты. Всего проанализированы данные 1246 сессий. НЭ наблюдались во время 56,8% проанализированных сессий и в течение 24 ч после 21,5% сессий. В большинстве случаев они были легкими и не влияли на желание продолжать стимуляцию. Отказ от проведения ТМС из-за НЭ был зарегистрирован в шести случаях (3,3% участников), в трех из которых наблюдалась выраженная головная боль (ГБ) после стимуляции. Во время стимуляции чаще всего регистрировались сонливость (32,4%) и боль в области стимуляции (20,3%). Наиболее частым НЭ в течение 24 ч после ТМС была ГБ (13,3% сессий), в связи с чем описаны ее характеристики и предложены возможные диагностические критерии.</p></sec><sec><title>Заключение</title><p>Заключение. НЭ могут возникать во время сессии ТМС и после нее, в большинстве случаев они являются легкими.
