It covers the current views of the role of chronobiological mechanisms in the regulation of sleep and awakening in PD. The results of a follow-up of 20 patients (aged 58.56+8.24 years) diagnosed with PD without dementia with subjective nocturnal sleep disorders (disease history was 4.44+3.46 years; PD stage, 2.5+0.47) are given. In addition to antiparkinsonian drugs, melatonin was given in a dose of 3 mg/day. The efficiency of the therapy was evaluated by clinical assessment procedures before and 4—8 weeks after administration of the drug. The therapy outcome was better sleep characteristics in 17 (85%) patients, as evidenced by specialized questionnaires. The positive therapeutic effect as judged by the changes in Parkinson's disease sleep scale scores was shown by improvement of subjective sleep quality, reductions in awakening difficulties, in the total number of nocturnal awakening, in the degree of restlessness in bed, which mimics akathisia, and in the frequency of awakenings that were associated by the patients with urges to urinate (p&lt;0.05).</p></abstract><trans-abstract xml:lang="ru"><p>В статье приведены особенности клиники и патофизиологии нарушений сна и бодрствования у пациентов с болезнью Паркинсона (БП). Освещены современные представления о роли хронобиологических механизмов в регуляции сна и бодрствования при БП. Представлены результаты наблюдения 20 пациентов с диагнозом БП без деменции с субъективными нарушениями ночного сна (возраст — 58,56+8,24 года, длительность заболевания — 4,44+3,46 года, стадия БП — 2,5+0,47). В дополнение к противопаркинсоническим препаратам назначался мелатонин в дозе 3 мг/сут. Оценка эффективности терапии по методикам клинической оценки проводилась до назначения мелатонина, а также через 4—8 нед приема препарата. Результатом терапии явилось улучшение характеристик сна по данным специализированных опросников у 17 (85%) пациентов. Положительный эффект терапии по динамике показателей шкалы оценки сна при БП (PDSS) проявился субъективным улучшением качества сна, уменьшением затруднений при засыпании, сокращением общего количества ночных пробуждений, снижением выраженности двигательного беспокойства в постели, напоминающего акатизию, сокращением частоты пробуждений, которые пациенты связывали с позывами к мочеиспусканию (р&lt;0,05).</p></trans-abstract><kwd-group xml:lang="ru"><kwd>болезнь Паркинсона</kwd><kwd>инсомния</kwd><kwd>мелатонин (мелаксен)</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Parkinson’s disease</kwd><kwd>insomnia</kwd><kwd>melatonin (melaxen)</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">&lt;div&gt;&lt;p&gt;Нодель М.Р., Яхно Н.Н.
