HAMS diagnosis was established in the presence of two or more exacerbations per year without treatment or one exacerbation in patients receiving pathogenetic treatment in the presence of concomitant manifestations of the activity of the pathological process on MRI. Before initiation of cladribine therapy, the median exacerbation rate was 2.3 per year, i.e., 34 patients per year had a total of 78 exacerbations.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Only two clinical exacerbations were registered (one each in the first and second year) during two years of treatment among all 34 patients, the average frequency of exacerbations was 0.05 per year; in three cases, a subclinical exacerbation was noted - new Gd+ lesions on T2-weighted images on MRI, the total number of events was 0.15 per year. No significant adverse events were reported.</p></sec><sec><title>Conclusion</title><p>Conclusion. Cladribine tablets are a highly effective and safe treatment option for HAMS. </p></sec></abstract><trans-abstract xml:lang="ru"><p>Цель исследования – изучить в общеклинической практике эффективность и безопасность кладрибина в таблетках в качестве средства терапии иммунной реконституции высокоактивного рассеянного склероза (ВАРС).</p><sec><title>Пациенты и методы</title><p>Пациенты и методы. Два полных курса терапии получили 34 пациента с ВАРС, которые наблюдались в Московском центре рассеянного склероза в 2018–2021 гг. Диагноз ВАРС устанавливался при наличии двух и более обострений за год без терапии или одного обострения на фоне патогенетической терапии при наличии сопутствующих проявлений активности патологического процесса на МРТ. До начала терапии кладрибином средняя частота обострений составила 2,3 случая в год, т. е. у 34 больных в год суммарно было 78 обострения.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. В течение двух лет терапии среди всех 34 больных было зарегистрировано только два клинических обострения (по одному в первый и во второй год), средняя частота обострений составила 0,05 в год; в трех случаях отмечалось субклиническое обострение – новые Gd+-очаги на Т2-взвешенных изображениях на МРТ, всего событий – 0,15 в год. Существенных нежелательных явлений не отмечено.</p></sec><sec><title>Заключение</title><p>Заключение.
