Examinations were made in 60 patients with type 1 DM and 60 with type 2 DM who were divided into groups according to the presence of CI. The Montreal Cognitive Assessment (MoCA test) was used to evaluate CI. Glycemia and its variability and the level of glycated hemoglobin (HbA1c) were analyzed. MRI brain images were segmented using the FreeSurfer program and the Recon-all software package. Brain functional MRI was performed using paradigms (for Broca's, Wernicke's and primary motor areas).</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Patients with type 1 DM and CI were found to have reduced volumes of the entire white matter and the right hippocampus; those with type 2 DM had decreased gray and white matter volumes. The functional activities of the primary motor and Broca's areas were reduced by 50% in patients with types 1 and by 80% in those with type 2 when conducting the tasks. A relationship was established between disease duration, patient age, glycemic variability index, MoCA test results, and decreases in the volumes of the hippocampus and white and gray matter and in the activity of motor areas in types 1 and 2 DM.</p></sec><sec><title>Conclusion</title><p>Conclusion. Gray and white matter atrophy and impaired motor area functional activity are more significant in type 2 DM. These changes are associated with CI and glycemic variability.</p></sec></abstract><trans-abstract xml:lang="ru"><p>Сахарный диабет (СД) — метаболическое заболевание, которое сопровождается поражением головного мозга, проявляющимся когнитивными нарушениями (КН).</p><p>Цель исследования — изучение структурных и функциональных изменений головного мозга у пациентов с СД в зависимости от наличия КН. Пациенты и методы. Обследовано 60 пациентов с СД 1-го типа и 60 с СД 2-го типа, которые были распределены в группы в зависимости от наличия КН. Для оценки КН использовали Монреальскую шкалу когнитивной Дисфункции (МоСА-тест). Анализировали гликемию и ее коэффициенты вариабельности, уровень гликированного гемоглобина (HbA1c). Сегментацию изображений головного мозга по данным магнитно-резонансной томографии (МРТ) проводили с помощью программы FreeSurfer и программного пакета Recon-all. Функциональную МРТ головного мозга выполняли с применением парадигм (для зоны Брока, Вернике и первичной моторной зоны).</p><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. У пациентов с СД 1-го типа и КН выявлено уменьшение объема белого вещества в целом и гиппокампа справа, у пациентов с СД 2-го типа — объема серого и белого вещества. Функциональная активность первичной моторной зоны и зоны Брока при проведении заданий у пациентов с СД 1-го типа была снижена на 50%, а с СД 2-го — на 80%. Установлена связь между длительностью заболевания, возрастом пациентов, индексом вариабельности гликемии, показателями MoCA-теста и снижением объема гиппокампа, белого и серого вещества и активности моторных зон при СД 1-го и 2-го типов.</p></sec><sec><title>Заключение</title><p>Заключение. Атрофия серого и белого вещества головного мозга, нарушение функциональной активности в моторной зоне более значительны при СД 2-го типа.
