The investigation enrolled 105 depressive patients without psychotic symptoms, who were observed to develop symptoms of SSRI-induced apathy during SSRI antidepressant monotherapy for at least 6 months. The patients were randomized to groups: 1) 35 patients were switched to milnacipran (Ixel 50–100 mg/day) with simultaneous antidepressant replacement; 2) 35 patients were prescribed milnacipran with a 2-week gradual discontinuation of the previous SSRI; 3) 35 patients were switched to combined therapy with milnacipran 50–100 mg/day and sulpiride (Eglonyl 50 mg/day). The latter was prescribed to test the hypothesis for the benefits of combined correction of SSRI-induced apathy syndrome. The duration of the investigation was 3 months. Changes in the condition of the patients were assessed during their visits before and 1, 2, 4, 8, and 12 weeks after changing therapy. The efficiency of antidepressant therapy was studied using the 21-item Hamilton Depression Rating Scale (HDRS-21) and the Clinical Global Impression (CGI) rating scale; the changes in the severity of asthenic manifestations were examined according to a subjective asthenia assessment (Multidimensional Fatigue Inventory (MFI-20)). The UKU side effect rating scale was used to verify side effects.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. A significant (p&lt;0.05) pronounced thymoleptic effect of various treatment regimens (simultaneous and gradual replacement with monnacipran monotherapy, as well as a milnacipran-sulpiride combination) was confirmed at week 4 of treatment. There was a reduction in SSRI-induced apathy syndrome when switching to treatment with milnacipran or milnacipran-sulpiride in 71.4–80% of patients. Different therapy replacement regimens showed varying changes in the reduction of heterogeneous asthenoapathic symptoms. A favorable safety profile was established for both monotherapy with milnacipran and its combination with sulpiride.</p></sec><sec><title>Conclusion</title><p>Conclusion. The antidepressant milnacipran (up to 100 mg/day) exhibits a high efficacy and a good tolerance in case of SSRI-induced apathy syndrome, as well as by a pronounced thymoleptic effect, including when combined with the antipsychotic sulpiride (50 mg/day).</p></sec></abstract><trans-abstract xml:lang="ru"><p>При приеме селективных ингибиторов обратного захвата серотонина (СИОЗС) у 5–20% пациентов возникает синдром СИОЗСиндуцированной апатии.</p><p>Цель исследования – изучение возможности и преимуществ перевода пациентов с СИОЗС-индуцированной апатией на лечение селективным ингибитором обратного захвата серотонина и норадреналина.</p><sec><title>Пациенты и методы</title><p>Пациенты и методы. В исследование включено 105 пациентов с депрессией без психотической симптоматики, у которых на фоне монотерапии (длительностью не менее 6 мес) антидепрессантами  из группы СИОЗС отмечалось развитие симптомов СИОЗСиндуцированной апатии. Пациенты были рандомизированы в группы. Пациентов 1-й группы (n=35) переводили на милнаципран (Иксел, 50–100 мг/сут) с одномоментной заменой антидепрессанта. Во 2-й группе (n=35) милнаципран назначали с постепенной (2 нед) отменой предыдущего СИОЗС. Пациентов 3-й группы (n=35) одномоментно переводили на комбинированную терапию милнаципраном (50–100  мг/сут) и сульпиридом (Эглонил, 50 мг/сут). Последний назначали для тестирования гипотезы о  преимуществах комбинированной коррекции синдрома СИОЗС-индуцированной апатии. Длительность исследования составляла 3 мес. Динамику состояния пациентов оценивали во время визитов до смены терапии и на 1-й, 2-й, 4-й, 8-й и 12-й неделях. Эффективность антидепрессивной терапии изучали по шкале депрессии Гамильтона (HDRS-21), шкале общего клинического впечатления (CGI); динамику выраженности астенических проявлений – по субъективной шкале оценки астении (МFI-20). Для верификации побочных эффектов  использовали шкалу побочных эффектов UKU.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. К 4-й неделе лечения подтвержден значимый (р&lt;0,05) выраженный тимолептический эффект различных схем терапии (одномоментная и постепенная замена на монотерапию милнаципраном, а также комбинация милнаципрана с сульпиридом). Редукция СИОЗС-индуцированного апатического синдрома при переходе на лечение милнаципраном или милнаципраном с сульпиридом зарегистрирована у 71,4–80% больных. При разных схемах замены терапии обнаружена различная динамика редукции гетерогенной астеноапатической симптоматики. Установлен благоприятный профиль безопасности как монотерапии милнаципраном, так и его комбинации с сульпиридом.</p></sec><sec><title>Заключение</title><p>Заключение. Антидепрессант милнаципран (до 100 мг/сут) характеризуется высокой эффективностью и хорошей переносимостью при СИОЗС-индуцированном апатическом синдроме, а также выраженным тимолептическим эффектом, в том числе при комбинации с антипсихотиком сульпиридом (50 мг/сут).</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>СИОЗС-индуцированная апатия</kwd><kwd>лечение</kwd><kwd>милнаципран</kwd><kwd>сульпирид</kwd></kwd-group><kwd-group xml:lang="en"><kwd>SSRI-induced apathy</kwd><kwd>treatment</kwd><kwd>milnacipran</kwd><kwd>sulpiride</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Статья спонсируется компанией «Фарм Фирма «Сотекс».
